Reporting Incidents

At Ariel Assist, safety comes first. If something goes wrong, big or small, it must be reported quickly. This includes accidents, mistakes or times when something almost went wrong

Reporting helps us:

  • Fix problems fast
  • Learn from what happened
  • Stop it from happening again

Some incidents must also be reported to the NDIS Commission within 24 hours.

These include:

  • The death of a person with disability
  • A serious injury
  • Abuse or neglect
  • Unlawful sexual or physical contact or assault
  • Sexual misconduct, including grooming
  • Use of restrictive practices that are not allowed or not in a behaviour plan

Your report helps keep everyone safe and cared for.

Keeping Children Safe

We work hard to keep children safe. If we think a child (aged 0–15) may be in danger or harmed, we must tell the Department of Communities and Justice (DCJ).

This includes concerns about:

  • Physical or sexual abuse
  • Neglect or emotional harm
  • Danger to themselves or others
  • A carer not being able to keep caring

Why it matters? Your report:

  • Protects the people we support
  • Helps us follow the law
  • Keeps everyone safe and well

If you see something worrying – speak up. It’s important and it makes a difference.
